Basic information needed to apply for Social Security Disability

There are several pieces of documentation disabled employees need to provide when they apply for Social Security Disability benefits.

Individuals who have a disability that prevents them from working may be eligible for Social Security Disability benefits. Applicants should be prepared to provide the Social Security Administration with documentation such as medical records and recent earnings information. Questions applicants may need to answer

In addition to this documentation, there are several other pieces of information SSD applicants may be asked to provide during the application process. For example, disabled workers may be asked some or all of the following questions as well as additional ones dependent on their circumstances:

  • Their Social Security number, name at birth, current name and gender
  • Their birthday and location of their birth
  • Whether or not they have used another Social Security number in the past
  • Whether active military service was provided prior to 1968 and if any Federal or civilian agency has provided a monthly benefit in the past
  • Whether they are eligible to receive retirement benefits, such as a pension or annuity, from other employment
  • Whether they are married at present and their spouse’s name, date of birth and Social Security number

Applicants may also be asked about any children they have under the age of 18 as well as children ages 18 to 19 who are in secondary school. Additionally, applicants may be asked about their prior employment history for the current and former year as well as if they expect to receive payment from an employer after becoming disabled.
